Objectives: Children with general diseases (AE) show an increased need for dental treatment compared to healthy children of the same age. In addition, therapy under intubation anesthesia (ITN) is necessary, especially for children with general illnesses. This often leads to very long waiting times, which is due to the low capacity of dental treatments for patients with AE. The aim of the present retrospective study was to compare the caries experience, the need for treatment and the waiting time in children with AE at a university hospital (AP) and healthy control patients (GP) in the clinic and practice. Methods: A total of 325 (179m, 141f, 5 unknown) patients (263 AP, 62 GP) were recorded who received at least one tooth restoration in ITN at the clinic or in a private practice in the period from 2013 to 2019. 76 had cardiac disease (HE), 18 had tumor disease (TE), 54 had syndromic disease (SY), and 115 had other diseases (SE). 62 children (34 clinic, 28 practice) were healthy. In addition to age, gender and general medical history, the waiting time between the initial examination and anesthetic treatment, dmf-t/DMF-T values, as well as the significant caries index (SiC) and the Caries-Restauration Index (KSI) were determined. The children with caries were divided into two age groups (preschool children (<6 years); n=165 and school children (>6 years); n=160) based on their age. The descriptive evaluation was carried out with the program SPSS 26.0. Significant differences were tested using ANOVA (Welch test) (p<0.05). Results: The mean dmf-t/DMF-T values of the children with early childhood caries (preschool children) were found to be higher in comparison to those of the school children in all patient groups. The lowest mean dmf-t/DMF-T value was independent of the age group the GP (clinic) with the highest value being in the TE preschoolers and the SY school children (p>0.05).The highest SiC index was 17.00±2.00 in the group of preschool children with TE and 13.5±3.8 in the school children with SY, with the lowest SiC index being 9.0±0.8 and 9.0±1.2 among GP school children, respectively. The lowest KSI was 0.0 % within the TE preschool children and 5.0 % in the HE school children. The highest KSI was 41.08 % in the group of SY school children. The waiting time for treatment in ITN was higher in the children with AE than in the GP, with the peak value of the preschool children here at 6.3±4.5/4.8 months per child in the groups of HE and SY lay. In the preschool children with mixed dentition however, the maximum value of 15.0±19,8 was in the GP group (clinic). Among the school children with pure primary dentition, the children in the SY group had the longest waiting time with 45.5±58,7, whereas the children with mixed dentition in the GP group (clinic) had to wait the longest (7.9±13.4). In the school children with pure permanent dentition, the waiting time was the longest for the HE group with 26.8±36.2 (p<0.05) and one patient with TE who had to wait 36 months. Conclusion: The study showed a high caries experience and a low Caries-Restauration Index (KSI) as well as a longer waiting time in children with AE compared to GP. A long waiting time underscores the importance of improving dental care options for children with AE. It seems urgently necessary for these children to have intensive prophylactic dental care in order to minimize or, at best, avoid ITN treatments with all the associated unpleasant side effects and risks.
